As for a vacation... I'm biased because I love the beach, so going somewhere with a beach would be awesome. I'd think about this person, though. If they like being with only one or two good friends, maybe a camping trip together! If they like being around a lot of people and feeling normal for a change, maybe an amusement park or circus or fairgrounds. If they like learning, there's lots of big cities where you can hit museums and art galleries and guided tours of a city all in one day!
Sometimes just asking can be the most thoughtful thing! Rather than doing something on the fly without checking with the other person, if they're not that fond of surprises. Everyone's different.
Besides, if you ask now, you'll know better for next time! Taking their feelings into account like this already tells me you care.
I really don't want to ask them. It makes me feel insensitive if I can't figure out what they'd like. So I'm just trying to get some small idea of what I could do.
Well but... I can think of only a few circumstances where anyone can guess
what someone else would like without asking them. I don't think you should
feel bad about it at all! Is this for some special occasion, like a
birthday, that you're trying to put so much into surprising them? Otherwise
I think working so hard to make someone's wish come true - that you know is
their wish - is really admirable.

Oh! This one's a little more dicey, but would it be possible for someone else to ask for you? It'd have to be someone who you trust to communicate well, otherwise there could be some innocent misunderstanding going on. Alternatively, maybe a letter if you're too nervous to ask them face to face.

Well, I can understand not trusting someone enough for that. I didn't mean
to imply you shouldn't talk to them at all, but just about this. It still
sounds like it'll be hard to hide any of this is if you live with them. I
